FUE leaves dot-like scars approximately the diameter of the extraction tool, usually less noticeable as hair grows. Risk factors include extraction density, evenness of distribution, individual scarring tendency and preferred hair length.

Keene et al. (2018) note that tightly clustered extraction sites can cause patchiness and increase the risk of impaired local blood supply. Residual density of 20–30 FU/cm² can give a see-through appearance.
People prone to abnormal scarring have a higher risk. Marks are more visible with short or very short hair. Thinning caused by overharvesting cannot be fundamentally reversed by another operation.
